Prompt for deletion reason when a project is deleted

At the moment, when a squirrel deletes a project from whatever state, or when a project manager deletes a project from the new state, the deletion reason field is made visible in the project page form, but is left blank.

It would be helpful to prompt for a deletion reason as an additional step before the final deletion "are you sure?" prompt.

Further comment. At the moment this is the message that is displayed for "are you sure?"

Is there a better way to do this that displays the project title and author and projectid? That would have likely avoided deleting some projects that might ought not to have been deleted.
