[P4] GitHub Hygiene: Create issues for remaining gaps

Task

After VSCode audit (P1-T4/T5) and feature parity audit (P2), create GitHub issues for any remaining work discovered.

If VSCode extensions are incomplete (expected)

Create issues for each missing component:

• [VSCode] Complete opensin-code-vscode package.json
• [VSCode] Implement opensin-code-vscode extension logic
• [VSCode] Complete kairos-vscode package.json
• [VSCode] Implement kairos-vscode extension logic

If any services/commands have gaps

Create issues for each incomplete implementation

Validation

All gaps discovered during P1-T4/T5 and P2-T1/T2/T3 have corresponding GitHub issues

No additional gaps found during audits: VSCode extensions functional (both have packageManager: bun@1.3.12), all services/commands have real implementations, feature flags properly gated, A2A agent URLs OK (only emails, no dashboard URL violations), stub directories removed, package-lock.json files removed. All work tracked in existing issues.
