idd-edit batch 無法觀測 gh-egress tier-floor refusal（exit 13）—— #272 DA N-1 follow-up

Problem

#272 的 gh-egress net item 4（reply layer-3 tier floor：type=reply + points-from=user-pasted token 且 attested=light → exit 13）是 verb-agnostic 的——edit 改一則既有 reply comment 時同樣會觸發。但 idd-edit 的 batch exit contract（SKILL 的 R5 refuse 段）目前只認 R5-refusal（exit 4）：明文「不套 gh-egress 碼帶」，floor refusal（13）在 batch 迴圈中既不進 edited: 也不進 refused:，最終 exit code 也不會反映。

出處：#272 verify round 1 Devil's Advocate finding N-1（無 lens 覆蓋、未測試、未規範）。

idd-edit 的 batch 語意把 gh-egress refusal band（≥10，含 13）納入 per-comment outcome 分類：floor / privacy / mention refusal → 該 comment 記為 refused:（附 wrapper stderr 一行），batch 繼續（warn-continue），最終 exit 反映有 refusal。附 drift-guard 斷言。

Actual

exit 13 在 idd-edit batch 中是未定義行為——不出現在任何 outcome bucket，caller 無從觀測 floor refusal 發生過。
