Goto Exchange Admin Center and select Organization -> add-ins

Select Add

On 2016 you may need to download the manifest.xml file from file
https://outlook.sdrop.com/manifest.xml

Edit the app and make it Mandatory and always enabled.

I wonder if during install we choose add from URL instead of add from file, does that make a difference in the error that we're getting when opening the add-in 🤔
