README: how to update on each host, and how to unstick a stale one

Codex uses `codex plugin marketplace upgrade` then `codex plugin add`; Claude
Code uses `/plugin marketplace update`. Both are spelled out with a
version-check, and the Codex section documents clearing a cached copy or a
shadowing manual install.
