Plan mode loses answered clarifying questions when re-entering thread

When in plan mode, if the AI asks multiple clarifying questions and you:

1. Answer question 1
2. Leave the thread (e.g. to ask another AI thread for advice)
3. Return to the thread to answer question 2

The answer to question 1 is lost. The plan restarts as if no questions were answered.

Expected behavior: Previously answered clarifying questions should persist across thread re-entries. The plan should accumulate answers incrementally.

Actual behavior: Returning to the thread resets the clarifying questions entirely... It re-asks from question 1 as if nothing was answered.

Reproduction:

1. Start a task that triggers plan mode with 2+ clarifying questions
2. Answer the first question
3. Leave the thread, open a different one
4. Return to the thread
5. Observe: it starts over from question 1, previous answer is lost
