thinking blocks missing without settings.json override #89

See anthropics/claude-code#30958. Seems like the API by default stopped returning the actual thinking content. This will manifest as thinking blocks in the transcript that look empty. Should clarify either in the docs or the generated transcript that this doesn't actually reflect the fact that claude "skipped thinking", and hint at the settings.json workaround in the claude-code repo issue.
