Preserve chosen editor focus when delayed initial autofocus runs #200

Opening quick edit can redirect status-note typing into the title when the first dialog animation frame is delayed. In CAPTCHA PR #195 CI 40579557476, LARGE-01 fails because Add remains disabled. The trace shows the note input focused at 746363ms, then its text appended to the title at 746402ms. useModalA11y unconditionally focuses the first field in a deferred animation frame.

Acceptance:

• Preserve focus already chosen inside the dialog or deliberately moved elsewhere before initial autofocus runs.
• Preserve normal initial focus, keyboard trapping and lifecycle cleanup.
• A deterministic regression fails against the existing hook and passes with the correction; run actual persona workflows at their existing desktop/phone sizes with zero retries.
• Keep the original CI failure; no timeout/retry workaround.
