Summary

Refs #137. Spec-first companion: HQBase/hqbase-site#59

ChatGPT connections to /mcp/full end after the one-hour access token expires. ChatGPT requests
only the scopes in the WWW-Authenticate challenge (mail:read mail:write mail:send). It never asks
for offline_access, so Better Auth issues no refresh token.

#137 proposes to add offline_access to the MCP challenges. MCP SEP-2207 (Final) says a resource
server SHOULD NOT do that, and that the authorization server decides refresh-token issuance by its
own policy. HQBase is also the authorization server, so this change applies the policy there.
The challenges and the protected-resource metadata do not change.

Changes

  • worker/auth/mcp-offline-access.ts (new): before Better Auth handles
/api/auth/oauth2/authorize (GET or form POST) or /api/auth/device/code (form POST), add
offline_access to the scope parameter when all of these are true:
    • every resource is /mcp or /mcp/full;
    • the request has a scope without offline_access, and no sig (Better Auth signs
      continuation queries, and a change would break the signature);
    • the client exists, is not disabled, registered the refresh_token grant type, and allows
offline_access. This follows the SEP-2207 rule to give refresh tokens only to clients that
      say they can store them.
  • worker/routes/index.ts: pass /api/auth/* requests through this policy before Better Auth.
  • CHANGELOG.md: add a "Fixed" entry.

The consent page already shows offline_access as "Stay connected until you revoke access".
A person who approved the MCP scopes before this change sees the consent page again, with this line.
The Mail API (/api/v1, /api/v2) does not change. Its clients request offline_access themselves.

Tests

test/integration/worker/mcp-offline-access.test.ts (new) runs the real flows through Better Auth:

  • For /mcp and /mcp/full: read the challenge, authorize with exactly the challenge scopes and
PKCE, approve the consent, and exchange the code. The consent query and the token include
offline_access, a refresh token is issued, tools/list works, and a refresh grant returns an
    access token that works.
  • A client without the refresh_token grant keeps its requested scopes and gets no refresh token.
  • A Mail API resource (/api/v2) keeps its requested scopes.
  • An MCP device authorization request stores offline_access in its scope.

With the policy turned off, the three positive tests fail and the two negative tests pass.
The existing tests that check the unchanged challenges and metadata still pass.

Summary by CodeRabbit

  • New Features
    • Eligible clients that support refresh tokens can stay connected to MCP services after their access token expires. This applies to MCP connections and requests, without changing Mail API authorization scopes.
    • The consent page describes this as “Stay connected until you revoke access.”

ChatGPT requests only the MCP challenge scopes, so it never asks for
offline_access and its connection ends when the access token expires.
Add offline_access to MCP authorization and device requests from
clients that registered the refresh_token grant. Apply it as
authorization-server policy and keep the resource challenges and
metadata unchanged, as MCP SEP-2207 recommends.

The worker adds offline_access to eligible MCP authorization and device-code requests. Integration tests cover refresh-token issuance and use, and verify that ineligible clients and non-MCP resources do not receive the added scope.

MCP clients that omit scope may not stay connected after token expiry. Confirm the omitted-scope behavior before relying on this path; the remaining risk is narrow.

Comment thread worker/auth/mcp-offline-access.ts
Better Auth requests the client's registered scopes when scope is
omitted. Dynamic registration stores offline_access with them, so these
requests already get a refresh token. Add a test for that path.
