Set a value for a reference with a `set` marker, such as `set[cooldown=30]`, `set[stale-after=0]`, or `set[vulnerability-level=high]`. The comparison items such as `ignore[stale<90]`, the bare `ignore`, `ignore[stale]`, and `ignore[vulnerable]` are gone. Closes #496.
