Proposal for a new refinement interface which allows for combinations of degree elevation and knot insertion.

- allows subdivisions into any number of sub-elements
- allows degree elevation
- generalizes "two-scale" relations to mean relations between a coarse and a subdivided- and/or degree-elevated-space
- implements explicit algorithms for Bernstein polynomials

  • So far, updates to canonical spaces, univariate B-splines, and UnivariateBSpline and TensorProduct two-scale operators
  • The tests for TensorProductTwoScales have been updated to account for the new structure
