リリース: PAT失効時のフィードバック送信エラーと起票失敗の診断を修正

フィードバック受付時にPATの認証と起票先へのアクセスを確認し、失敗した場合はHTTP 503を返します。アプリの既存の送信エラーダイアログで表示できます。

GitHubの起票失敗ではステータスとリクエストIDを記録します。レート制限時はRetry-Afterの秒数・HTTP-dateと制限解除時刻に従って再試行を遅らせます。

対象: #50。本番ブランチとの差分はこの修正の9ファイルです。検証済みのdevと同じツリーをmasterからのリリースコミットにしています。348件のテスト、型チェック、lint、両環境のビルドが成功しました。

Binding・secret・queue構成・KV・R2・Cronの変更はありません。masterへのマージで既存の本番デプロイを実行します。
