Bump version to 3.15.0 and write release notes - #141

Conversion semantics moved from 7 to 8, so plans saved by 3.14.x are refused; like v3.13.0 for the last semantics change, this is a minor version. The notes cover the ASCII-is-already-UTF-8 rule and the seven fixes since v3.14.5, list what scripts may notice, state that the four- corpus audit runs on this commit with results recorded after tagging, and say plainly that the executables are not code-signed.
