+        Use this form to propose a long lived CNCF User Group (UG) for TAB review. A UG serves a documented, outstanding cloud native domain gap and, once approved, reports to the TAB.
+
+        Before submitting, review the [CNCF User Group Governance](https://github.com/cncf/tab/blob/main/governance/ug-governance.md). Discussion oriented groups should instead apply as [CNCF Community Groups](https://github.com/cncf/communitygroups#how-to-apply), and short term, deliverable focused work should be proposed as an initiative.
