The index parses all transcripts in one transaction and propagated TooLarge from the file, line and message limits, so a single over-limit file (a 518 MB Codex rollout on one store) rolled the whole run back. The file is now refused alone and counted as refused; other errors still abort. Release 0.62.16.

… the run

The index parses every transcript inside one transaction and propagated
TooLarge from the file, line and message limits, so one over-limit file
rolled the whole run back. On one store a 518 MB Codex rollout (file cap
256 MB) kept `lore index` failing after the oversized-session fix,
leaving the index at 295 sessions.

index() now skips a file whose parse returns TooLarge, counts it as
"refused" in the result, and keeps going. Every other error, including
UnsafePath, still aborts the run.
