Completes the remaining documentation scope of #37.

  • adds a task-oriented inbound Message Streams guide
  • ships client and worker examples using stable message IDs
  • explains replay-safe cursors, duplicates, continue-as-new, signals, and outbound Workflow Streams
  • validates the examples against the public PHP API

Closes #37
