test: rescan 自愈回归与 pytest integration marker 机制

What & why

补齐 #6 的回归覆盖缺口：

1. tests/test_rescan_selfheal.py：钉死 _rescan_sources 的 stale SYNCING 自愈契约（>3×lease 重置 ACTIVE + sync_error 留痕、fresh SYNCING 与 ACTIVE 不受影响）——生产曾因 SYNCING 卡死触发该修复，此前零回归护栏（三路审计缺口清单高风险项）；
2. pytest.ini 注册 integration marker 并文档化约定：本仓库 tests/ 套件默认即集成（依赖真实 PostgreSQL，make test-backend），无 DB 的纯单测放 tests_oauth_patch/（不在 testpaths）——回应 [P1] Harden scheduler, background jobs and lifecycle reliability #6 验收「external I/O mocked + 集成测试单独标记」（HTTP/LLM/DNS 已全 mock，DB 维度按此口径文档化处置而非对 130 个文件做标记翻新）。
