Fix current-lane projection and recovery churn

Summary

• replaces stale active-run/readback surfaces with a single current-lane/run-lease projection model
• demotes cleanup-complete unleased lanes from CLI/WebUI current lanes using terminal ledger authority
• records phase-goal recovery evidence and blocks repeated same issue/phase/error automatic continuation churn
• surfaces continuation recovery state in CLI/WebUI/macOS and adds macOS progress percent readout
• retires old active_lease/activeRuns/owned_active external fields in favor of run_lease/current_lanes/leased_run
