📖 docs: spell out ACMM (AI Codebase Maturity Model) on first use

Feedback from @mrbobbytables: ACMM was never spelled out. First use on the default page now reads "AI Codebase Maturity Model (ACMM)" with a one-line gloss and links to the level matrix and the arXiv paper.
