codex exec不接受--ask-for-approval（CI 示例全部修正）；[profiles.x]写法现在会报错 → 改为独立<name>.config.toml；codex mcp add语法；mcp-server已移除 →app-server；退役模型 gpt-5.4 → gpt-6 系列<name>/SKILL.md目录（单文件会被忽略），角色文件放.claude/agents/；.claude/rules/用paths:；只有 exit 2 的 hook 会阻断过时信息
