The directory structure seems wrong. The ISSUE_TEMPLATE directory needs to live inside a .github directory (yes, even in the .github repo)
Also could be good to add a config.yml file with blank_issues_enabled: false to avoid blank reports.
But overall I love this idea! Thanks!
