Summary

Bump the CLI, npm package, and Windows runtime versions to 1.13.1.
Update the README version badge and release notes for the Claude ask_user status fix.
Document that current PreToolUse claude hooks are recognized while genuine legacy hooks remain repairable.

Validation

Targeted ask-user hook regression test passed.
Full local test suite: 28 passed, 0 failed.
npm pack --dry-run.
Bash syntax checks, CLI version output, and git diff --check.
