When AUTHZ_API_URL or BILLING_BASE_URL are not set (e.g. self-hosted deployments without Warden/Aether), providers now return undefined instead of crashing. Entitlements and authz modules guard accordingly.
Type inference tests were using toBeDefined without parens (no-op). Rewrite as compile-time type-level checks.
