⚡ Bolt: Scope lesson progress queries to active course/lesson IDs

💡 What: Optimized the query for user lesson progress on the course index page, student dashboard page, and certificate pending lessons page.

🎯 Why: Previously, these pages retrieved the user's entire historical lesson progress. For users with a large history, this results in fetching hundreds/thousands of unneeded rows, causing large database payloads and high memory consumption.

📊 Impact: Reduces database payload size, saves database CPU time, and decreases memory usage. Scopes queries to only the displayed courses or lessons.

🔬 Measurement: Checked via Prisma logs/analytics showing that the query now includes a WHERE lessonId IN (...) clause, matching exactly the visible lessons.
