**Problem**: When embedded commands failed, `mdcmd` would crash with a
stack trace instead of handling errors gracefully.

**Changes**:
- Add `CommandError` class to track failed commands without raising exceptions
- Modify `async_text()` to catch `CalledProcessError` and return `CommandError`
- Update `process_path()` to collect errors and report them to stderr
- Update `main()` to exit with code 1 if any commands failed
- Add tests for error handling behavior

**Behavior**:
- Commands that fail no longer crash with stack traces
- Failed commands are reported gracefully with exit code and output
- Other commands continue to execute in concurrent mode
- Exit code 1 when any command fails, 0 when all succeed

Update to use `utz>=0.21.1` which includes the fix for `text()` handling
`None` return values from `output()`.
