fix(ci): keep the review checkout clean for Pullfrog's checkout_pr

Summary

The Submission review workflow writes the reviewer's prompt to submission-review-prompt.md in the repository root, leaving an untracked file. Pullfrog's checkout_pr tool refuses to switch branches over a dirty tree, so the agent had to delete the file before it could check out the pull request (it recorded this in its learnings). The prompt file is now added to .git/info/exclude, and the step asserts the tree is clean.

Validation

• Reproduced the step in a fresh clone: with the prompt written, git status --porcelain is empty.
• actionlint passes; zizmor reports nothing.
• Re-running the review on an open submission after merge confirms checkout_pr works without the workaround.
