Complete outstanding installed verification from merged work

Complete the installed verification that was still outstanding in docs/LIVE-GATE.md before #35 consolidated that document. These rows are unfinished work, so track them here rather than keep a historical ledger in the runtime reference.

Use current main after the cleanup is merged, install the exact commit, and record CLI version, real session entry point, action, and observed artifact for both Claude Code and Codex. Runner checks and self-reports do not clear these rows.

• feat(pstack): promote GPT-6 Codex families to stock, new first-run defaults, README rewrite #16: GPT-6 families and first-run defaults; setup and mixed panel.
• fix(hooks): make pstack opt-in on Claude Code #19 / docs: note that Codex runs the opt-in session hook #20: opt-in behavior with and without naming pstack.
• Merge open-pstack 1.5.0 (Cursor pstack 0.15.5) #22: open-pstack 1.5.0, setup step order, and Grok 4.7; setup and mixed panel.
• Agent monitor: live canvas of running agents, lanes, and messages #24 / feat(monitor): focus on pstack sessions and show what each agent is doing #25: optional lane journal; installed run with journal enabled and absent, with psf-monitor compatibility. Previous evidence was a checkout build only.
• refactor: rename to pstack-flex, add intake and diff-behavior, add the local and live gate docs #26: distribution rename, intake, and diff-behavior; opt-in, setup, issue brief, and intended/accidental behavior comparisons.

All were previously marked not run from an installed candidate. The current OpenCode cleanup has its own candidate gate in #33 and #35; these existing debts are not evidence that the new candidate passes.
