Implement runtime-only MySQL schema discovery and selection. Profiles no longer require a persisted database; Test Connection discovers visible schemas; each run requires an explicit runtime schema. Work follows the approved design in
docs/superpowers/specs/2026-08-10-runtime-schema-selection-design.md.
