Description of the idea or improvement

Add /copilot doctor as a deterministic read-only command in issue and PR conversations. Reuse existing setup/doctor policies and typed semantic read ports where their trust and credential roles match. Inspect the effective runtime configuration, installed workflow contracts, issue/SDD prerequisites, and approval policy accessibility/validity. Report pass, warning, blocked, unavailable and not-applicable separately, with one concrete next action and links. Actual successful reads are evidence only for those reads; never infer unavailable grants, write permission, credential health or producer attestation. No model invocation, credential disclosure or repair mutation. Preserve existing issue/PR status and locale behavior.

Current limitations or challenges

During the previous live round, recommendation assessment failed with policy-unavailable although the policy was valid. Diagnosis required opening workflow logs and using local doctor. /copilot diagnose is an AI investigation, not a deterministic runtime configuration audit. Developers should obtain safe evidence in the conversation without supplying tokens or installing a local CLI.

Expected impact

A maintainer can identify a missing prerequisite, inspect its source and retry after fixing it. The command never edits files, labels, variables, secrets, projects, checks or reviews; it publishes one correlated localized reply. Missing and unreadable policy are distinguished from invalid JSON. PR identity remains exact and linked issue is not inferred from matching numbers. Repeated deliveries update the same reply. Define a proportional numeric budget of at least 40 distinct cases across pure policy, read orchestration/errors, authorization, replay, locale, sanitization, workflows and documentation; preserve existing repository coverage budgets.

Second deep dogfooding round. Create and verify the SDD before implementation through the installed feature workflow and in-progress label. Consult specs/catalog.json and the setup/doctor, comment-automation and semantic-publication owners. Follow .agents/skills/product-specification/SKILL.md and specs/_template.md. No new configuration flag or permission expansion; unreadable evidence remains unavailable. Runtime PAT and local setup operator PAT are distinct: do not pretend the latter exists in an Action run. Ask blocking product or architecture questions through SDD Q.
