feat: establish open-source workflow foundation

Summary

• add immutable owner-scoped workflow revisions and execution run/attempt history
• connect approval, dispatch, interrupt, deletion, and revision conflict handling
• add architecture, privacy, permission ADRs, roadmap, and release metadata
• add real Playwright approval and refresh-recovery flows with a contributor-safe runner

Verification

• build passed
• TypeScript and ESLint passed
• 39 test files / 288 tests passed
• Playwright 2/2 passed with zero retries
• independent review found no release blockers

Release scope

This prepares Roundtable for an open-source beta. Import/export, observability, full pause/resume, and community templates remain follow-up phases.
