Document the missing token for unresolved models.
The resolver can produce PROFILE_SOURCE.UNRESOLVED when the profile, agent definition, and configured default provide no model. That source produces no modelSource, and taskHeader omits the token. Therefore, “Every agents task card shows” is too broad. The existing default entry already identifies Pi’s default model.

Derive modelSourceToken from the routing result.
sessionProfileRoutingAt reads the target routing first. modelSourceToken then rereads mutable pin and declaration files. A change between these reads can make the token name a different layer from the layer that supplied the model.
The follow/live path also differs from resolveInheritedProfile for Git worktrees. When no usable pin exists, resolveInheritedProfile skips the declaration and falls back to the global profile. The token can still find an unversioned declaration and return "declaration". Checking that a declaration exists does not prove that its modelProfiles supplied this agent's model.
Return the winning source together with the routing result, or otherwise pass source metadata from the single resolver read into modelSourceToken. Do not reread the pin or declaration files when generating the token.

Field report by @zhalker on #1064: disk edits below a winning binding or frozen profile no-op silently, and diagnosing it required running
loadAgentsConfigcold outside the process. @matraket generalized it: with #1827 every session freezes at startup, so this staleness is the default state, not an edge case. Recording the winning layer in every task card makes it visible for free —resolveAgentProfilealready computed the source.Test plan
