Bound POSIX cleanup after disconnect.

If a shim stops the supervisor with SIGSTOP, the supervisor cannot run its disconnect handler. stop() then waits indefinitely for the supervisor, so timeout and cancellation cannot complete.

Add a bounded escalation that terminates the owned process tree and bounds the cleanup wait. Killing only the supervisor can leave descendants running.

Review comment at @packages/create/src/environment.ts around lines 109 - 110:
Update stop() cleanup after subprocess.disconnect() so a stopped supervisor
cannot block timeout or cancellation indefinitely: add a bounded escalation that
terminates the owned process tree, including descendants, and ensure the wait
for cleanup also has a finite bound. Reuse an established process-tree
termination mechanism if available; do not rely on terminating only the
supervisor.
