PAT失効時にフィードバックの送信エラーを返す

フィードバック受付後にGitHubの認証で失敗すると、キュー内で起票できないままアプリには受付成功を返していました。キュー投函前にOCTOKIT_PATで起票先リポジトリへのアクセスを確認し、PAT未設定・失効、GitHubのHTTPエラー、5秒のタイムアウト、Issues無効時はHTTP 503とcallableのunavailableを返します。アプリの既存の送信エラーダイアログで失敗を表示できます。

GitHubの起票失敗ではHTTPステータス、リクエストID、検証エラーの項目を構造化ログに記録します。投稿本文や認証情報は記録しません。レート制限時はRetry-Afterの秒数・HTTP-dateとx-ratelimit-resetに従ってキューの再試行を遅らせます。

成功応答はキュー受付完了を示します。受付後に発生するPAT失効や起票時の検証エラーまでは同期的に保証しません。受付ごとにGitHubへの確認リクエストが1件追加されます。

検証: npm run typecheck、npm run lint、npm test -- --runInBand（21スイート・348件成功）、devとproductionのwrangler deploy --dry-runが成功しました。

Binding・secret・queue構成・KV・R2・Cronの変更はありません。PATの更新や本番デプロイは含みません。
