Stacks on #357 (base style/kill-ai-slop; retarget to main after #357 merges).

One toolbar row: status FilterToggle with counts (only the needs-attention count is red when >0), provider HeroUI Select with icon + count, search (~240px), Refresh credits + Add account on the right. Wraps on narrow screens.
Removed: stat row (accounts / available / needs attention / in flight), the '1–8 / 8' text, the provider FilterToggle.
Clear filters now also resets the provider filter.
FilterToggleOption.label accepts ReactNode. Frontend only; no API change.
