fix(sdk, cli): terminal-state no-ops, bounded pm next receipts, lean refusals with typed claim holders - #1637
Three GitHub-reported agent-UX and SDK defects, each reproduced on current source, fixed with a measured before/after and a negative control, and closed in the tracker with escape class and gate evidence.
What changes
Identical terminal updates keep their closure clocks (SDK: forced identical closed-state update resets closed_at and updated_at #1628, pm-gh1628).update routed any payload naming the close status to the closure writer without checking whether the item was already closed, so a forced identical closure re-stamped closed_at and updated_at on every retry while the same payload on an open item was a no-op. routeCloseStatusUpdate now reads the stored item first: when the requested close status, trimmed reason and completed_at already describe it, only the remaining fields take the ordinary skip-no-op update path and the result reports terminal_state_unchanged; a bare identical update returns no changed fields without --force. A different reason is still a repeat closure behind the existing force gate. commitItemMutations reuses update(), so transactions inherit the fix. Helpers live in src/sdk/lifecycle/repeated-terminal-state.ts to keep update.ts under the line ceiling.
pm next discloses the whole response under its ceiling and encodes tags losslessly (Agent UX: pm next prints ~6.8 KB against its 640-token budget — side sections unbudgeted, escaped tag JSON, all-null columns (the #1401/#1603 fixes reached pm context only) #1627, pm-gh1627).filters.token_budget reported 640 while nothing enforced it, and --token-budget replaced the universal default ceiling instead of bounding the selection beside it, so the complete response carried no cost receipt. The flag is no longer a read-output cost alias; a requested selection budget now audits the complete envelope (read_output with budget_source: default, budget_tokens, estimated_tokens beside truncation.ready_budget) unless an enforced intent receipt already discloses it, and filters.token_budget echoes the request or null. The TOON codec records omitted null columns as null_column_index instead of repeating the full column_order list (decoders still accept the legacy field), so the compact table wins for ready and containers and every section list-encodes tags. On the real tracker: 9,148 → 8,718 bytes, 182 → 76 escaped-quote sequences.
Lean refusals drop constant boilerplate and the claim holder is typed (Agent UX: refusals repeat the headline and constant boilerplate; --lean makes text errors larger #1610, pm-gh1610). The SDK declares the constant refusal boilerplate once (PM_GENERIC_REFUSAL_GUIDANCE, PM_RECOVERY_ECHO_FIELDS, isGenericRefusal*, projectLeanRecoveryPayload) and both lean projections consume it: lean text keeps the headline, next steps, specific requirement/rationale, verifier findings and specific recovery; lean JSON drops generic required/examples and the invocation echo on top of title/why. A single-line message is no longer repeated under What happened in any mode. already_claimed_by carries item_id, claimed_by, claimed_at and claim_source through PmCliErrorContext, the JSON envelope and the classification; text renders Held by: <holder> (<source> since <time>). Claim conflict: text 798 → 719 bytes, lean text 813 → 494, lean JSON 968 → 648. A property test over six refusal codes asserts lean is never larger than default and shares no constant line.
Contracts and docs
SDK surface snapshot acknowledges NextResult.filters.token_budget (number | null), PmTableCellEncoding.null_column_index, the PmCliErrorContext holder fields and the new exports (resolveItemOwnershipConflict, the lean boilerplate helpers, CloseRouteContext).
Contracts snapshot drops the stale --output-budget alias declaration on next.
docs/COMMANDS.md, docs/AGENT_OUTPUT_ENCODINGS.md, docs/AGENT_UX_CONTRACTS.md and docs/AGENT_GUIDE.md describe the terminal-state receipt, the two next budget scopes, the lean refusal contract and the holder fields.
Validation
node scripts/run-tests.mjs test: full fast suite green on the merged branch (see the last commit's evidence in the tracker).
…al state
An update naming the close status was routed to the closure writer without
checking whether the item was already closed. With force, the writer
re-stamped closed_at and the store stamped updated_at on every retry, so a
repeated identical closure moved the recorded closure time (GH-1628); the
same payload on an open item was a no-op. commitItemMutations reuses
update(), so transactions shared the defect.
routeCloseStatusUpdate now reads the stored item first. When the requested
close status, trimmed reason and completed_at already describe it, the
remaining fields take the ordinary skipNoop update path and the result
reports terminal_state_unchanged; a bare identical update returns no
changed fields without --force. A different reason is still a repeat
closure behind the existing force gate. The helpers live in
repeated-terminal-state.ts to keep update.ts under the line ceiling.
Regression tests cover the forced identical update, the unforced identical
update with a padded reason, other fields applied without re-stamping
closed_at, the preserved repeat-closure route, and a mixed-batch
commitItemMutations retry; three of the four update tests fail without the
fix.
…ncode null columns by position
pm next reported filters.token_budget 640 while nothing enforced it, and
--token-budget replaced the universal default ceiling instead of bounding
the selection beside it, so the complete response (9,148 bytes on this
tracker) carried no cost receipt at all (GH-1627). The TOON codec lost the
compact table for ready and containers because omitting one all-null
column emitted the full fifteen-name column_order list, so those sections
fell back to escaped tag JSON.
--token-budget is no longer a read-output cost alias for next; the
universal default ceiling keeps binding and a requested selection budget
audits the complete envelope, so read_output reports budget_source default,
budget_tokens and estimated_tokens beside truncation.ready_budget unless an
enforced intent receipt already discloses the cost. filters.token_budget is
the requested selection ceiling or null. PmTableCellEncoding records
omitted null columns as null_column_index and decoders still accept the
legacy column_order.
Measured on the real tracker: pm next --limit 5 drops from 9,148 to 8,718
bytes and from 182 to 76 escaped-quote sequences, every section
list-encodes tags, and --token-budget 640 --json carries the receipt
(budget 6000, estimated 4119). The contracts snapshot drops the stale
--output-budget alias on next.
… fields
Refusals printed a single-line message twice (headline and What happened;
title and detail in JSON), kept the generic requirement, rationale and
pm --help examples under --lean, and echoed the caller's own argv, so a
lean text refusal was larger than the default one (813 against 798 bytes
for a claim conflict). The holder of a claimed item existed only inside
the sentence (GH-1610).
The SDK declares the constant boilerplate once (PM_GENERIC_REFUSAL_GUIDANCE,
PM_RECOVERY_ECHO_FIELDS, isGenericRefusal*, projectLeanRecoveryPayload),
and both lean projections consume it: lean text keeps the headline, next
steps, a specific requirement or rationale, verifier findings and specific
recovery; lean JSON drops generic required/examples and the recovery echo
on top of title/why. A single-line message is no longer repeated under
What happened in any mode. already_claimed_by carries item_id, claimed_by,
claimed_at (latest assignee change in history) and claim_source
(claim|assignment) through PmCliErrorContext, the JSON envelope and the
classification; text renders Held by: <holder> (<source> since <time>).
resolveItemOwnershipConflict is public for packages.
Claim conflict: text 798 to 719 bytes, lean text 813 to 494, lean JSON 968
to 648; JSON grows by the typed holder fields. A property test over six
refusal codes asserts lean is never larger than default and that no
constant line is shared by every lean output.
…ence
Each item records its reproduction, the before/after measurement, the
negative control and the gates that ran, carries escape class and gate
evidence for the defect evidence gate, and links its files, docs and a
linked test that passed. docs/COMMANDS.md documents the terminal-state
receipt, the two next budgets and the lean refusal contract. The SDK
surface snapshot acknowledges the NextResult, PmTableCellEncoding and
PmCliErrorContext changes and the new exports. Changelog regenerated.
Stacked on #1635 (base will move to main once #1635 merges). Three items: GH-1628 terminal-state no-op, GH-1627 bounded pm next receipts and the null_column_index codec, GH-1610 lean refusals with typed claim holder fields.

Bug Fixes
Repeating an identical update to a closed item now preserves its closure and update timestamps. Changes to other fields can still be applied without altering the closure time.
Claim-conflict errors now include structured details about who holds the item and how they acquired it.
pm next clarifies selection-budget reporting and includes a response-size audit when a budget is requested.
Compact table output preserves null-column positions while reducing unnecessary metadata.
This pull request changes refusal rendering, ownership-conflict details, pm next budgeting and table encoding, and repeated terminal-state updates. It also updates public SDK declarations, documentation, tests, changelog entries, and issue records.
Lean text and JSON omit generic boilerplate and echoed recovery fields while retaining specific recovery data and holder evidence. The CLI passes its lean setting to error rendering. Tests cover the text and JSON projections and ownership fields.
filters.token_budget is null unless a selection budget was requested. A requested budget applies to ready selection and triggers a whole-response read-output audit unless an in-budget context receipt is present.
Compact encoding records omitted null-column positions in null_column_index. Decoding restores column order and continues to support legacy column_order metadata.

The change has bounded remaining issues: concurrent reopen operations can defeat a repeated close request, required blank reasons can bypass validation, and lean refusals can omit specific rationale or misstate ownership origin. These warrant owner awareness and correction, but no broad outage or data-loss risk is established.

Explanation
#1628 is addressed by the terminal-state routing and direct, transaction, and mixed-batch regression evidence. #1610 is addressed by lean projections, typed holder fields, and size/content tests. #1627 has the receipt and compact null-column changes, but the reviewed test evidence covers a small two-item fixture and does not include the required large-fixture pm next output-budget contract test. The evidence also does not establish the issue's side-section include and limit contract under that fixture.
Resolution
Add and pass a large-fixture #1627 contract test. The test must verify the complete pm next envelope, side-section include and limit behavior, TOON tag arrays, and the null-column receipt. Fix any implementation failures that the test exposes.

Preserve terminal-state timestamps and history for idempotent close-status updates while still applying unrelated field mutations.
Read the persisted item before close routing and detect matching status, trimmed reason, and completion time.
Return a terminal-state-unchanged receipt for identical updates, including forced retries, and delegate remaining fields to the normal update path.
Keep different-reason repeat closures behind the existing force gate and cover transaction retries.
Separate pm next selection limits from whole-response accounting and make compact table encoding lossless with smaller metadata.
Report a nullable requested selection budget and retain the universal output ceiling for the complete response.
Emit a read-output receipt for requested selection budgets unless an intent receipt already discloses the response cost.
Encode omitted null-column positions instead of repeating full column order, while preserving legacy decoder compatibility and compact list/tag representations.

Compute null column positions in one pass.
Each omitted null column runs columns.indexOf(column). Compact encoding of a table with many all-null columns therefore costs about nullCount × columns.length comparisons. Build the positions in a single pass over columns, using the existing nulls set.

Update the compact encoding that builds null_column_index to scan columns once
and use the existing nulls set to collect positions, avoiding a separate indexOf
search for each null column while preserving the corresponding order of
null_columns.
Add a case that exercises the universal output ceiling in the next audit test.
The test "reports a null selection budget unless requested and audits the complete envelope under the universal ceiling" runs next --token-budget 5000 with two short items. Its read_output.estimated_tokens <= read_output.budget_tokens assertion holds for any small envelope, so the test does not exercise the ceiling. The 5000 value bounds only the ready rows. It is not the limit that this assertion checks.
Add a case whose complete next envelope is large enough to test read_output.budget_tokens. Assert that read_output.estimated_tokens stays within read_output.budget_tokens and that read_output.budget_source is "default". Also assert whichever omission or compaction receipt the implementation reports for that case. Size the fixture against the default ceiling, not the 5000 selection budget.

Add a forced repeat-closure test with a different completedAt.
The new repeated-closure tests omit completedAt. A closed item that receives a different completion time is not treated as a repeat, so the supplied time is written to completed_at. No test covers that path. A regression that discarded the supplied time would leave the existing tests passing.
Add a test that force-closes an already closed item with a different valid completedAt. Assert that the persisted completed_at equals the supplied value.

…keep code-specific lean rationale, and report an unknown holder origin
CodeRabbit raised six findings on #1637 and hosted coverage found four
uncovered branches. Every finding is fixed here and every branch is
now covered.
Terminal-state no-op (GH-1628):
- A whitespace-only --close-reason on an already-closed item answered
terminal_state_unchanged even with governance.require_close_reason on,
while the same request on an open item is refused. The predicate now
sends a supplied-but-blank reason to the refusing close route only
while a reason is required; an optional blank reason stays a no-op.
- The classification read holds no item lock. If another writer reopened
the item before the field-only write, the request dropped its status.
The helper now checks the status the locked write returned and hands a
reopened item to the closure writer (closeRoutedItem, now shared with
the ordinary route).
- docs/COMMANDS.md now says other requested fields move updated_at and
write history only when they change a stored value.
Lean refusals and typed holder (GH-1610):
- projectLeanErrorEnvelope dropped every why. It now drops only the
generic rationale, matching lean text and the documented contract.
- Mixed example lists kept the generic `pm --help`. The new SDK helper
withoutGenericRefusalExamples filters each generic entry for text and
JSON alike.
- An unreadable or silent history reported the holder as `assignment`.
claim_source / ItemOwnershipConflict.source now say `unknown`. The SDK
surface snapshot acknowledges the union widening; both types are
unreleased and were introduced on this branch.
Coverage (GH-1627 and the above): in-process tests now cover the next
selection-budget audit (camel and snake spellings, intent suppression),
null-column restore without recorded positions, holder rendering without
a time or origin, a changed versus repeated --completed-at, and the
routed not-found refusal. Before this, those paths only ran in spawned
CLI processes.
This also merges the updated #1635 head.

…d cover every reason outcome deterministically
Hosted coverage missed the weighted policy's "relevance over pN ready
work" branch. The test asserted it only when the seeded scores happened
to put the P2 log first, and in CI they did not.
The ranking helpers (resolveNextRankingPolicy, applyNextRankingPolicy,
describeRankingPolicyReason, NEXT_RANKING_POLICIES, NextRankingPolicy)
now live in src/sdk/query/next-ranking-policy.ts. next.ts re-exports
only the two public names, so the published ./sdk/query surface is
unchanged (0 classified changes) and no _testOnly export grows. A table
test covers each reason outcome: empty queue, floor agreement and
disagreement, a weighted leader passing over P0 work, and a weighted
leader whose only higher-priority peer is in progress.
The preceding merge regenerated the contract snapshot. That fixes the
static gate, which was red because `pm next` now emits
invalid_argument_value for an unknown policy. The merge also brings in
the updated #1637 head. Notes are recorded on pm-gh1618.
The lean projections now filter generic examples per entry through
withoutGenericRefusalExamples, which left the all-or-nothing predicate
with no caller. Hosted coverage flagged it as an uncovered function. It
was added unreleased on this stack (absent from main and v2026.10.30),
so it comes out of the ./sdk, ./sdk/contracts and ./sdk/runtime barrels
instead of staying as a dead public export. The SDK surface snapshot
acknowledges the removal.
This also merges the #1635 head, which closes the shared-unset-fields
coverage gap here as well.
… module to its item
The error-code catalog takes each emitting command's name from the file
that throws. Moving resolveNextRankingPolicy into next-ranking-policy.ts
made the catalog list a fake next-ranking-policy command for
invalid_argument_value, and hosted static flagged the catalog as stale.
The resolver goes back into next.ts, the command that owns the refusal.
The new module now holds only the pure ranking and reason helpers.
The tracker-context-quality gate also refused next-ranking-policy.ts
because no item owned it. It is now linked to pm-gh1618, whose review
note is corrected to match.
This also merges the #1637 head, which removes the superseded
isGenericRefusalExamples predicate and closes the shared-unset-fields
coverage gap.
This PR's commits now land through #1638, which carries the whole stack (#1635 → #1637 → #1638) to main in one CI cycle. Its CodeRabbit findings were fixed in fe37f78 and 33aeb3c. GitHub marks this PR merged when #1638 merges.
Lands the agent-UX stack (#1635, #1637, #1638): same-command corrections and bounded receipts, terminal-state no-ops, bounded pm next, lean refusals with typed claim holders, the pm next ranking policy, the close claim-principal fix (pm-8xgg7i), the development-dependency advisory overrides (pm-irviv0), and the pm-changelog 2026.10.31 grammar census.

Three GitHub-reported agent-UX and SDK defects, each reproduced on current source, fixed with a measured before/after and a negative control, and closed in the tracker with escape class and gate evidence.
What changes
updaterouted any payload naming the close status to the closure writer without checking whether the item was already closed, so a forced identical closure re-stampedclosed_atandupdated_aton every retry while the same payload on an open item was a no-op.routeCloseStatusUpdatenow reads the stored item first: when the requested close status, trimmed reason andcompleted_atalready describe it, only the remaining fields take the ordinary skip-no-op update path and the result reportsterminal_state_unchanged; a bare identical update returns no changed fields without--force. A different reason is still a repeat closure behind the existing force gate.commitItemMutationsreusesupdate(), so transactions inherit the fix. Helpers live insrc/sdk/lifecycle/repeated-terminal-state.tsto keepupdate.tsunder the line ceiling.pm nextdiscloses the whole response under its ceiling and encodes tags losslessly (Agent UX: pm next prints ~6.8 KB against its 640-token budget — side sections unbudgeted, escaped tag JSON, all-null columns (the #1401/#1603 fixes reached pm context only) #1627, pm-gh1627).filters.token_budgetreported 640 while nothing enforced it, and--token-budgetreplaced the universal default ceiling instead of bounding the selection beside it, so the complete response carried no cost receipt. The flag is no longer a read-output cost alias; a requested selection budget now audits the complete envelope (read_outputwithbudget_source: default,budget_tokens,estimated_tokensbesidetruncation.ready_budget) unless an enforced intent receipt already discloses it, andfilters.token_budgetechoes the request ornull.
